Documents to be Submitted

The following documents should be attached in original or as attested copies verified by Notary. School leaving certificate must be translated to Slovak Language according to the procedure mentioned below. The documents listed below are to be submitted directly to the following address:

List of documents to be submitted to the Application: 

  • Application Form. Click here to fill out the on-line Application Form. Select the Study programme you wish to apply for (General Medicine, GM or Dental Medicine, DM). Please make sure that you provide your correct email account! Once you have filled out the Form please press the "Submit". The completed application form will return to us. The form is going to be processed and completed then a printable form is going to be sent back (as email attachment) for you to sign it.
  • Medical certificate (not older than 3 months, prepared in English or in Slovak), what has to be signed and stamped by your family doctor verifying that the applicant is physically and mentally able to complete studies at university and able to fulfill the requirements of services in the chosen study program.
  • Confirmation of the payment of your Entrance Exam Fee: 40 Euro (non refundable, directly payable to the account of the Faculty – see banking coordinates below).Important note: entrance exam fee will be paid by us in case a Declaration Form is signed expressing that the applicant would like to be represented by our agency.
  • Copy of Passport ; EU citizen may submit the copy of ID card as well.
  • School Leaving certificate. (In case it is not yet available at the time of Application, you have to submit it – following the requirements below – by the time of Entrance Exam at the latest.)
  • According to legal requirements in Slovakia applicants must submit a notary authorised copy of the so called apostillized certificate and its notary authorised translation by a court translator to Slovak language. The Court Translation and its notary authorization must be done in Slovakia 
    The “Apostille” (meaning certification) – based on the original school leaving certificate– must be received in the country the certificate has been issued (usually by the Ministry of Education).
    Click here for details about the competent authorities. In case the country is not on the list, the applicants must have the documents "superlegalized".
    Exemption from Apostillization: applicants completed their secondary education in the following countries: Croatia, Hungary, Germany, Poland and Romania (Ukraine) are exempted. They have to submit, however, a notary verified copy of the school leaving certificate and its official translation to Slovak language according to the system mentioned above.

    The submitted documents will be immediately transferred to the Study Department of the Faculty of Medicine.

  • Signed Brief Curriculum Vitae (CV) in English

  • One Passport size photograph (signed on the back)

  • Verified copy of the birth certificate. If it is issued in other language than English, notary authorized translation to English or Slovak Language is required.

 

Failure of submitting the required documents will exclude student from registration.
